Description: THE SOCIAL WORKER IS RESPONSIBLE FOR ENSURING THAT CLIENTS AND THEIR CAREGIVERS RECEIVE THOSE SUPPORTIVE SERVICES APPROPRIATE TO EACH CLIENT AND CAREGIVER. THE SOCIAL WORKER FACILITATES AND ENCOURAGES EACH CLIENT TO FUNCTION AS INDEPENDENTLY AS POSSIBLE BOTH WITHIN AND OUTSIDE OF THE CENTER. Responsibilities: Gives input and reviews the programming and services received by each client, taking a leadership role in client advocacy. Performs intake and evaluation of each client to determine individual needs, general functioning level and appropriate placement in the adult day program. Completes all intake paper work in an accurate and timely manner. Facilitates team communication, keeping staff informed of any major changes in the client’s needs and/or home environment. Provides support to the client through individual, family and group counseling. Provides support to the caregiver through individual, family and group counseling. Facilitates client referral to other agencies, as appropriate, to meet clients' healthand social needs. Plans and facilitates activity groups for assigned program. Is involved in activities as schedule allows. Serves as the information and referral contact for the program, providing telephone information and on-site tours. Supervises and/or provides learning opportunities for social service students accepted for practicums within the center. (If applicable.) Communicates information to supervisor in a timely manner and consults with supervisor concerning difficult or unusual situations or ethical issues. Uses good judgment in decision making and communication with supervisor putting client's and caregiver's needs before personal or program consideration. Requirements: Master's or Bachelor's degree in Social Work, Counseling, Psychology or Gerontology from an accredited university. Current Ohio licensure as a Social Worker desired. Demonstrates ability to use a variety of counseling modalities including individual counseling, family counseling, group counseling and staff consultation. Ability to be flexible and to work collaboratively with other staff. Must have a car and valid Ohio driver's license. Must have understanding of the physiological, psychological, emotional and social development of older adults, especially those with medical/psychological disabilities. Knowledge of and ability to access community resources. Able to learn and use computer programs.
