Title: Case Manager
Hours Desired: Fulltime Hours
Work Hours: Generally 7:30-4:30 -May vary as needed
Hours/Wk: 40 hrs minimum 
Pay: $16.25
Description: Local non-profit organization is currently seeking a Case Manager for its Workforce Development department. A Case Manager will assist individuals with disabilities and other barriers to obtain and maintain employment by planning, implementing, and monitoring the progress of customers in programs and services. The Case Manager also coordinates services for program customers as needed and provides social service referrals to community partners. This involves but is not limited to; Write monthly Individual Plans of Employment (IPE) and complete monthly update reports for referring counselors. Act as a liaison between referring counselor, customers, their supports, and other pertinent staff. Meet with customers to ensure progression through programs and services, ensuring that they are meeting monthly IPE goals. Attend and participate in monthly interdisciplinary team meetings. Make recommendations concerning individual customers to referral sources, as needed. Record case notes on customer interactions and progress, and monthly staffing meetings into the Database. Conduct orientations for new customers and their supports. Enter new customers into database, and update records, as needed. Report recommendations to other team members and obtain feedback. Establish relationships with customers and their supports to ensure progress through programs and other services. Perform other related duties as assigned.
Requirements: At least a four-year college degree in any of the following fields: Psychology, Special Education, Rehabilitation and Counseling Services, or Social Work or related field. One to three years experience in counseling or social work with individuals with disabilities and other barriers to employment. Clinical training and/or case management experience in the vocational rehabilitation field is preferred. License and/or certification related to counseling or social work is helpful, but not required. Must maintain current CPR and First Aid certification. Must have valid OH driver’s license and personal car insurance. Must be able to problem solve in cases involving customer concerns and behavioral issues. Must have adequate computer skills to enter reports and case notes into system.
